Sign up and Activate your MyHealth account

Signing up for MyHealth is easy!

If you are the parent or guardian of a child under 13 years old, you may sign up with MyHealth Online to communicate with your child’s pediatric office regarding non-urgent matters by completing a Pediatric Proxy form.

Please mail the completed proxy form to:

or bring the completed proxy to your Pediatrician’s office.

Access is granted once the signed paper proxy form has been completed and processed by DMA. You will receive an email with your access code and can then begin using MyHealth. Please provide an email address that you check regularly.

Pediatric patients ages 13-18, the parent or legal guardian and adolescent must complete (and return in person) the completed “Adolescent Access Form”.

If you are over the age of 18 years, to sign up fill out an online application. You will receive an e-mail with an access code to complete the process. Once you have been notified that the processing of your request is complete, you may begin using MyHealth. Be sure to provide an e-mail address that you check regularly.

What can MyHealth Do for You?

MyHealth is an online patient portal that provides a password-protected, secure area on the Dedham Medical Associates website. With MyHealth you may view lab results and check certain portions of your health record. In addition, you may send a message to your doctor’s office, request an appointment or a prescription renewal and complete other tasks. MyHealth makes it easier for you to communicate with your doctor’s office and monitor your health records online. All transactions are secure and private.
